C2C is authorised to provide face-to-face case-management and therapeutic and support to clients in all Correctional Centres in Queensland.
our services support clients from Corrections to Community
Correctional Centre Intervention and/or Case-Management
We offer Face-To-Face, Virtual or Telephone Conferencing.
- Bio-psycho-social intake assessment.
- Targeted individualised treatment planning.
- Communication with legal professionals and key (family/friend) support personnel as required/requested.
- Relapse prevention.
- Addiction and co-occurring offending behaviour/s treatment programs, tailored to the individual’s outstanding treatment needs.
- Referral to complementary support services to assist with issues expressed by the individual, or barriers to achieving parole/bail.
Community-Based Support Post-Release.
- Case Management & Therapeutic Support:
- Addiction treatment;
- Relapse prevention
- Referral/s to supportive & complementary services (eg. Medical, Housing, Psychiatry, Allied health professionals, Rehabilitation Services);
- Pathology Testing;
- Consultation/liaison with Community Corrections.
- Attendance At Court, Probation and/or Parole Appointments.
Report Writing: For use in legal proceedings, parole or bail applications.
Letter of Engagement/Support (brief)
- Letter confirming client’s engagement with C2C (eg. correctional officer, DPP, legal representative).
- Intake & Biopsychosocial overview.
Treatment Plan (comprehensive)
- Intake & Biopsychosocial overview.
- Community-based treatment recommendations with evidence-based literature.
Complex Summary Reports (clients who have engaged in 6+ sessions)
- Intake Assessment & Biopsychosocial Overview.
- Attendance Dates & Times: including engagement & participation.
- Session Overview: key topics & learnings from each session.
- Final Summary & Recommendations: with evidence-based literature to support commentary by facilitator.
